Please download your Grant Application below. After completing your application, you may then email it, along with any supporting documentation,  to elsietrust18@gmail.com, or return it by U.S. mail to The Elsie K Nelson Trust, 6051 Glenbeigh Dr., Sylvania, OH 43560

 The Board of Trustees will meet each year on or about the following dates to review Grant Applications:

First Quarter-04/15

Second Quarter-07/15

Third Quarter-10/15

Fourth Quarter-01/15
